Insurance and Payment Information

The services provided by the office are contractual agreements between you and your child’s physician. We will be happy to help you obtain reimbursement from your insurance company, but you are responsible for payment to this office. Each month you will receive a statement showing your previous balance and any current services that have occurred along with the insurance company name and date billed. If you make arrangements with our billing department, we will be happy to assist you with any specific forms required by your insurance carrier. Please contact your insurance representative if you have any problems collecting reimbursement from your insurance company.

Required Forms for Insurance Providers

Should you have a carrier that may require a specific claim form, coordination of benefits form, accident details form, please be certain that you have furnished them with the requested information, in a timely manner. Many insurance providers will not pay without the completed forms.

Financial Issues

Our fees for professional services are based upon skill, judgment and time required to perform a given service and are comparable with those charged by other pediatricians in our area. Do not hesitate to ask our billing department any questions you may have about the fees.

Co-payments and co-insurance payments are expected at the time of service. We honor most major credit cards.
